| Purpose: |
The Memphis Plan is a ministry of the Church Health Center, which
is an ecumenical healthcare ministry. The Memphis Plan is a
program of primary care and limited hospitalization for low wage,
uninsured employees or self-employed individuals. Services
are provided on a volunteer basis by physicians in their offices,
labs, and hospitals. |
| Eligibility: |
An employer must have less than 200 eligible employees to participate
in the Plan. An employee is eligible to enroll if he or
she:
- makes under $10.00 an hour OR makes less than $400 a week
- works at least 20 hours a week
- is currently uninsured
- has completed three consecutive months of employment

| Fee: |
$45 per month ($10 employer, $35 employee)
$25 per month, for each dependent
$120 maximum amount per month, per family |
| Services: |
- Primary Care Benefits
- Preventive Health Services - Routine exam, and vision screening
- Sick Care - Office visits to your primary care physician, also
$5 per visit
- Subspecialty Services - Referral by primary care physician, also
$10 per visit
- Pharmaceutical Discounts - Discounts available at Ike's and
Super D
- Limited Hospital Services
- Emergency and surgical care up to a lifetime maximum of $50,000
